About the role

To provide early and targeted support to aid the recovery and resilience of single adults or childless couples with multiple vulnerabilities, working flexibly across the city supporting people in their own homes identified as ‘precarious’ housing, in locations of their choice and if/when they change accommodation. Demonstrating a commitment to safeguarding in line with policy and procedure you will support vulnerable adults to develop the knowledge and skills they need to better manage their lives, including accessing appropriate services, empowering them to build successful relationships within their community, improving health and well being and increasing levels of independence as they move through a prevention pathway model. 

Key Responsibilities:

  • Undertake effective needs and risk assessments with people referred to the service in accordance with the service’s eligibility criteria.
  • Develop co-produced, outcome focused support plans that reflect each customer’s personal strengths, goals and aspirations.
  • Deliver a flexible service, increasing or reducing the frequency and intensity of support as required and re-introduce support to dormant cases should any tenancy or support issues arise.
  • Support customers to sustain accommodation, improve health and well-being outcomes, develop skills required to live independently, including offering practical support, maximising income, budgeting, addressing debt.
  • Act as a community navigator, linking customers into local services and activities to establish networks of support and reduce social isolation. 

Any other duties as required by the role.
